The Salvation Army USA Southern Territory is seeking a highly skilled and compassionate Case Management Specialist to join our team. As a key member of our social services team, you will play a vital role in providing comprehensive case management services to clients participating in our life management program.
Job Summary:As a Case Management Specialist, you will be responsible for interviewing, accepting, and providing long-term, structured case management services to an assigned caseload of clients. You will work closely with clients to develop comprehensive program plans, evaluate their progress, and provide mentoring and counseling sessions to support their recovery.
Key Responsibilities:- Establish and implement comprehensive program plans for clients based on their unique needs and goals.
- Conduct regular meetings with clients to discuss their progress, feelings, and impressions, and provide guidance and support as needed.
- Collaborate with external case management professionals to ensure a holistic approach to client care.
- Prepare and maintain accurate and up-to-date records documenting client progress and services provided.
- Develop and implement strategies to reduce recidivism and improve client outcomes.
- Attend community meetings and events to educate others about our program and make referrals as needed.
- Bachelor's degree in Social Work, Behavioral Science, or a related field.
- Three years of progressively responsible experience in case management or a related field.
- Valid State Driver's License and CPR/First Aid Certification.
This position requires the ability to work in a shelter environment with exposure to physical health risks and unsanitary conditions. You will also be required to drive a vehicle to off-site meetings and home visits.
