About the Role:

We are seeking a skilled Nurse - Community Health Specialist to join our team at Providence Willamette Falls Medical Center. As a member of our nursing staff, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Deliver high-quality patient care in a community health setting
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Provide education and support to patients and their families to promote healthy behaviors and disease prevention
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

  • Graduation from an accredited nursing program
  • One (1) year of nursing experience
  • Current licensure as a Registered Nurse in the state of Oregon
