Job Summary:

The Urban League of Portland is seeking a highly motivated and dedicated Community Health Outreach Specialist to join our team. As a Community Health Outreach Specialist, you will play a critical role in connecting families with affordable health insurance options and providing connections to appropriate services to broadly address the health needs of program participants.

Key Responsibilities:

• Conduct outreach to recruit participants and educate community members of the current array of programs and services within the Urban League.
• Develop and maintain professional relationships and partnerships with community partners that are in contact with African American families who may be eligible for Oregon Health Plans, including schools, after-school programs, community clinics, local organizations, and faith-based communities.
• Assess clients' involvement in services, identify barriers to care and readiness to access care, including: client knowledge of eligibility status, health records, and other personal information required for Oregon Health Plan intake.
• Ensure compliance with all contract requirements including program performance data, reports, and documentation as required.
• Collect information and data using the highest degree of confidentiality and professionalism, as may be required for compliance reporting purposes.
• Maintain accurate, complete, and current written and database records/files.

Requirements:

• Bachelor Degree in Community Health, Social Work or related field.
• Minimum of three years of experience in community outreach, health education, or social services, working with and relating to African Americans.
• Community Health Worker (or other traditional health worker) Certification preferred but will train.
• Commitment to and knowledge of social justice issues and the dynamics of social change, the history and mission of the Urban League of Portland.
• Knowledge and understanding of working with low-income families and specifically within the African American community, particularly in light of the unique strengths and barriers in this community.
• Knowledge of emergency, social, education, domestic violence, health, human services, and information/referral systems.
• Demonstrated computer skills and proficiency in Microsoft Office Suites with an emphasis on Microsoft Word, Excel and Outlook programs.
• Must have a car for daily use, in-state insurance with your name on the policy and a valid Oregon state license, active for at least one year with an acceptable driving record.
