Job Title: Community Health Nurse

We are seeking a highly skilled and compassionate Community Health Nurse to join our team at Multnomah County (OR). As a Community Health Nurse, you will play a vital role in promoting health and wellness in our communities.

Job Summary:

The Community Health Nurse will provide comprehensive community and clinic health services, focusing on disease prevention, health promotion, and client self-care. This position requires strong assessment, planning, and evaluation skills, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Assess, plan, implement, and evaluate healthcare services to improve and promote healthy outcomes.
  • Provide regular screening, comprehensive assessment, and interventions for ongoing health-related issues, including pregnancy, postpartum care, infant child growth, and development.
  • Offer education through classes and individual care, ensuring clients receive the necessary information to make informed decisions about their health.
  • Coordinate care with other healthcare providers, ensuring seamless transitions and optimal patient outcomes.
  • Maintain clinical skills, staying up-to-date with the latest developments in adult, pediatric, OB, and internal medicine.
  • Seek expert advice on client issues and provide input to providers and nurses on client responses.
  • Complete documentation and charting as required by policy, ensuring accurate and timely records.
  • Assess patients referred to the program, including full assessments, psychosocial assessments, focused assessments, and options counseling, and provide the necessary interventions.
  • Mentor, coach, and offer clinical expertise to team members, promoting professional growth and development.
  • Delegate care to team members based on scope of practice and clinical skills needed by clients.
  • Ensure appropriate client follow-up and foster community partnerships to meet client needs.
  • Participate in quality improvement processes, contributing to team, role, and specialty workgroups.
  • Incorporate safe work practices with each task performed, actively participating in initiatives promoting a safe work environment.
Requirements:
  • Equivalent to a bachelor's degree in nursing or a related field.
  • License to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS).
  • Advanced knowledge, skill, and ability, with experience working with the Black/African American community, demonstrating an understanding of traditions, values, and customs.
  • A valid BLS/CPR card or the ability to obtain certification within 60 days of hire.
  • A valid state-issued driver's license.
Preferred Qualifications:
  • Demonstrated ability to professionally and effectively communicate orally and in writing with a variety of people, answering questions and explaining technical information.
  • Excellent written and verbal communication skills.
  • Experience managing competing priorities with multiple stakeholders.
  • Understanding of public health, chronic disease, and social determinants of health.
  • Deep understanding of disparities that inform equity-driven approaches to the work.
