Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as an Intake Specialist. In this role, you will be responsible for managing all aspects of the referral/intake process, collaborating with internal and external customers, and ensuring seamless transitions for patients seeking Behavioral Medicine Services.

Key Responsibilities
  • Manage all inquiries related to admission referrals, document on designated forms, and provide assistance to the HPH ER physician to determine eligibility for admission.
  • Conduct initial screenings, collect preliminary demographics, and verify insurance benefits.
  • Coordinate pre-admission activities with the BMU nursing unit, including validation of required legal paperwork and gathering of pertinent information.
  • Coordinate activities for patients accepted for admission, including completion of admission entry paperwork.
  • Maintain various logs of activities and participate in department/intake performance improvement initiatives.
  • May be cross-trained to be a medication nurse on the inpatient BH units.

  • Must pass physical, drug screening, and background check.
