Position Overview

AMIkids Baton Rouge, Inc. is seeking dedicated Case Managers to deliver essential services across various locations.

Role Summary

The Case Manager is tasked with guiding the Wraparound process for families involved in the program, ensuring timely and effective support. This role involves organizing and facilitating Family Team Conferences to align with the unique goals of each family.

Key Responsibilities

  • Maintain comprehensive knowledge of current service environments to provide a flexible and culturally sensitive service delivery system.
  • Coordinate and lead Family Team Conferences to foster consensus among family members and stakeholders in crafting suitable plans that address family needs while adhering to AMIkids standards.
  • Deliver services and interventions through a strengths-based approach, tailored to meet the individual needs of clients and ensure cultural competence.
  • Oversee the authorization and management of resources, including agency and community support, to fulfill client requirements.
  • Build and sustain collaborative relationships with family members, service providers, and relevant resources to effectively implement client plans.
  • Organize and schedule meetings, conferences, and travel to maintain efficient staff operations.
  • Conduct home visits or meet families in approved settings to provide support.
  • Operate program vehicles in compliance with transportation policies as necessary.
  • Participate in mandatory training and professional development to maintain relevant certifications and licenses.
  • Assist with special projects and perform additional duties as required.

Qualifications

  • Bachelor's degree in Psychology, Human Services, Social Work, or a related field; Master's degree preferred.
  • At least one year of relevant experience working with families and children; experience in Wraparound services is a plus.
  • Proficient computer skills.
  • Strong abilities in collaboration, negotiation, and problem-solving.
  • Ability to obtain and maintain necessary crisis intervention and physical restraint training as per state regulations.
  • Current CPR and First Aid certification from a recognized organization is required.
  • Willingness to complete external training as needed.
