About the Role:
The LPN is responsible for delivering high-quality patient care and performing technical skilled services in the comfort of the patient's home. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.
Your key responsibilities will include:
- Providing services within the scope of practice as defined by state laws governing nursing, in accordance with the plan of care, and in coordination with other healthcare team members.
- Completing progress notes for each visit and submitting them to the agency daily.
- Continuously observing and reporting patients' nursing needs, instructing family members in patient care, and maintaining a healthy environment for the patient.
- Observing, recording, and reporting patients' responses to treatment and changes in their condition to supervising nurses and physicians.
Requirements:
To be successful in this role, you must possess the following qualifications:
- Current LPN licensure in the state of practice.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR certification.
Estimated Salary Range: $60,000 - $80,000 per year
