Crossnore Communities for Children is seeking a dedicated Behavioral Health Care Navigator to join our team. This role plays a pivotal part in delivering tailored care management services to children with behavioral health needs and their families.

About the Role:

  • Deliver whole-person care to clients, addressing their behavioral, physical, and support system needs.
  • Oversee a caseload of 30 to 129 clients, ages 3-21, providing comprehensive assessments, care plan development, and multidisciplinary team coordination.
  • Arrange for round-the-clock emergency support, ensuring consistent access to care.
  • Conduct regular medication checks and support adherence with a focus on safe and effective medication use.
  • Connect clients to community resources for employment, training, social support, and personal development.
  • Educate members on self-care, wellness practices, and preventive care to foster long-term health.
  • Facilitate seamless transitions for clients moving between care settings, handling discharge plans, transportation, and home visit scheduling.
  • Guide families in accessing support services, building advocacy skills, and managing health-related challenges.
  • Establish clear policies for information sharing with clients, families, and community partners.

Requirements:

  • Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN).
  • Qualified Mental Health Professional credential required.
  • Knowledge of Child Welfare & Behavioral Health Systems.
  • Interpersonal & Communication Skills, especially in multicultural settings.
