About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Care Management team in a full-time capacity. As a Nurse Care Manager, you will play a vital role in developing, implementing, and evaluating individualized care plans for our patients. Your expertise will enable you to act as a patient advocate, liaise between patients, their families, and healthcare providers, and ensure that our patients receive the highest level of care in the comfort of their own homes and communities.

Key Responsibilities
  • Conduct initial assessments to determine patient needs, including medical, social, family, and living arrangements.
  • Develop, plan, implement, and evaluate ongoing care plans to meet individual needs.
  • Make home visits as needed to assess patient condition and modify visit frequency accordingly.
  • Notify physicians of changes in patient condition and coordinate care with other healthcare providers.
  • Accompany patients to physician appointments to ensure optimal healthcare and advocate for patient needs.
Requirements
  • Current RN licensure in Texas or eligibility for licensure under the Texas Board of Nursing rules for Nurse Licensure Compact (NLC).
  • At least one year of experience as an RN in a clinical role.
  • Effective written and verbal communication skills.
  • Ability to read, write, and comprehend English.
  • Basic computer skills and ability to navigate Electronic Medical Records.
  • Ability to triage patient needs, prioritize daily schedules, and think critically to solve problems.
