About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Care Management team as a RN Care Manager. As a RN Care Manager, you will play a vital role in developing, implementing, and evaluating individualized care plans for our patients.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal health outcomes.
  • Coordinate care with other healthcare providers, including home health, hospice, and nursing facilities, to ensure patients receive comprehensive and coordinated care.
  • Provide education and support to patients and their families to promote healthy behaviors and self-management of chronic conditions.
  • Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
Requirements
  • Current RN licensure in Texas or eligibility for licensure through the Nurse Licensure Compact (NLC).
  • At least one year of experience as an RN in a clinical role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to read, write, and comprehend English.
  • Basic computer skills and ability to navigate electronic medical records.
