Job Summary

We are seeking a highly skilled RN Stroke Navigator to join our team at Providence Oregon Regional Shared Services. As a RN Stroke Navigator, you will play a critical role in providing coordination between patients, primary care physicians, neurologists, and other specialists to ensure seamless care transitions.

This is a full-time position working day shifts. The RN Stroke Navigator will be responsible for performing in-hospital early post-discharge screening for medication compliance, healthy lifestyle, depression, medical complications, and caregiver burden. You will also serve as the patient's advocate, anticipating their needs and providing support throughout their care journey.

Responsibilities
  • Coordinate care transitions between patients, primary care physicians, neurologists, and other specialists
  • Perform in-hospital early post-discharge screening for medication compliance, healthy lifestyle, depression, medical complications, and caregiver burden
  • Serve as the patient's advocate, anticipating their needs and providing support throughout their care journey
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Provide education and support to patients and their families on stroke prevention, management, and recovery
Requirements
  • Bachelor's Degree in Nursing or obtained within 3 years
  • Oregon Registered Nurse License upon hire
  • National Provider BLS - American Heart Association upon hire
  • 5 years of related neurological clinical experience
Preferred Qualifications
  • Master's Degree in Nursing
  • NIHSS Certification upon hire
  • Stroke Certified Registered Nurse upon hire
  • American Association of Neuroscience Nursing membership and certification upon hire
  • 1 year of experience in an Ambulatory setting
  • Experience working with patients who have neurological conditions and their families
  • Experience working with a Joint Commission Certified Stroke Center
  • Experience leading formal educational forums/in-services
