About the Role

We are seeking a highly motivated and dedicated Transitional Employment Specialist to join our team at Aspire Health Partners. As a key member of our staff, you will play a vital role in providing essential functions to assess and place consumers in supported employment settings.

Key Responsibilities
  • Assess client's ability and willingness to work in various care settings
  • Locate and develop jobs for members according to their needs, interests, and abilities
  • Act as liaison with VR advocate to establish clients as VR eligible and refer them back to Aspire for supported services
  • Create and document case notes, training consumers on job sites
  • Process all VR paperwork and provide ongoing support
  • Initiate recommended changes for more effective operations and report regularly to clinical manager
Requirements
  • Ability to receive and maintain a Level II Background clearance
  • Acceptable Motor Vehicle Registration driver's license record in accordance with Aspire insurance company guidelines
  • High School Diploma; Bachelor's degree in a human services field preferred
