We are seeking a dedicated and compassionate Peer Support Specialist to join our team at Oaks Integrated Care. As a Peer Support Specialist, you will play a vital role in supporting individuals in their recovery journey, promoting self-determination, and fostering a recovery-oriented environment.
Key Responsibilities- Participate in team-based care and shared decision-making to support individuals in achieving their recovery goals.
- Support individuals in identifying and pursuing their self-chosen goals, and provide advocacy services to navigate the behavioral health system.
- Provide case management services, including referrals and linkages, to support individuals in accessing necessary resources.
- Assist individuals in identifying consumer supports and exploring social and recreational activities to promote independent living skills.
- Document all interactions with individuals and maintain accurate and up-to-date information in their electronic health record.
