Position Overview

Primary Role

The RN Long Term Service Program Coordinator is responsible for conducting assessments, evaluations, and planning for community support related to assigned long-term service programs. This role involves determining eligibility for personal care assistance (PCA) services and other in-home support options. The coordinator will also provide training and skill development for care attendants, ensuring they are equipped to support consumers effectively, while also empowering consumers through various educational topics that promote independence.

Key Responsibilities

  • Engage in the assessment and evaluation process to determine eligibility for GSIL's consumer-directed in-home support programs.
  • Provide consultations to individuals regarding health care concerns and available support options.
  • Collaborate with team members to assess the suitability of individuals for participation in attendant care programs.
  • Deliver orientation and training to consumers on the policies and procedures of the attendant care program.
  • Address health care management issues and assist in emergency medical planning, including participation in risk assessment meetings.
  • Regularly monitor and review participants' care plans, adjusting as necessary based on changing needs, including conducting six-month and annual reassessments.
  • Coordinate services while maintaining accurate consumer records in compliance with organizational policies.
  • Act as a resource for community members and staff regarding personal care options, community supports, and health-related issues.
  • Manage a caseload of PCA, NF Transition, and/or PCSP consumer-directed services.
  • Perform additional duties as assigned to enhance options for individuals with disabilities to live independently.
  • Collaborate with care managers from managed care organizations and CFI case managers to address consumer care needs.
  • Ensure accuracy and timeliness of ACE recruitment requests.
  • Participate in the development and delivery of departmental training sessions.
  • Assist consumers with transitions from nursing facilities and monitor their ongoing status.
  • Guide consumers in the hiring process for new ACEs, assisting with online applications and necessary documentation.
  • Facilitate care coordination efforts.
  • Engage in assigned committee work.
  • Conduct ACE training sessions as scheduled.
  • Maintain a flexible work schedule to meet departmental needs.
  • Enhance professional knowledge and skills through literature review and attendance at training workshops.
  • Support and adhere to GSIL policies, workplace regulations, and safety practices.

Qualifications

Registered Nurse with a current New Hampshire license and a minimum of two years of experience working with individuals with long-term health care needs. A strong commitment to promoting and maximizing independence for individuals with disabilities or seniors with chronic health care needs is essential.
