Job Summary:

Liberty Health is seeking an experienced Hospice Licensed Practical Nurse (LPN) to join our team. As a Hospice LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver compassionate and individualized care to our patients and their families.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and other necessary interventions.
  • Complete accurate and timely documentation of patient care, including progress notes and summary reports.
  • Collaborate with the Registered Nurse and other healthcare professionals to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.

Requirements:

  • Current LPN licensure in the state of employment.
  • One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
  • Ability to work independently and as part of a team to provide high-quality patient care.
  • Excellent communication and interpersonal skills.
  • CPR certification.
