Job Summary:We are looking for a compassionate and skilled LPN to provide direct patient care, including administering medications, wound care, and other necessary interventions. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and other necessary interventions.
- Complete accurate and timely documentation of patient care and progress.
- Collaborate with the Registered Nurse and other healthcare professionals to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in interdisciplinary team meetings to discuss patient care and progress.
- Current LPN licensure in the state of employment.
- One year of clinical nursing experience, preferably in an acute care medical/surgical setting.
- Ability to work independently and as part of a team to provide high-quality patient care.
- Effective communication and interpersonal skills.
- CPR certification.
