Key Responsibilities

The Hospice Nurse Practitioner will provide expert consultation in palliative care, symptom management, and supportive services tailored to the needs of hospice patients as referred by their physicians.

  • Conduct face-to-face visits with qualifying hospice patients within designated timeframes and communicate clinical findings to the Hospice Medical Director.
  • Collaborate with the Hospice Medical Director as necessary, inform the primary physician of recommended services, and work in conjunction with other healthcare providers.
  • Develop and coordinate the care plan, documenting issues, objectives, interventions, and patient/family responses to hospice services. Work closely with the patient/family, attending physician, and interdisciplinary team members to ensure comprehensive care.
  • Educate and guide patients and families in self-care practices when appropriate.
  • Maintain precise and relevant clinical documentation regarding the patient's status. Report any changes in patient condition and identified needs to the Administrator and Hospice Medical Director.

Qualifications

Experience Requirements

  • A minimum of five years of nursing experience; preferred backgrounds include clinical nursing in oncology, hospice, geriatrics, or intensive care.

License Requirements

  • Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
  • Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
  • Valid driver's license and vehicle insurance, with access to reliable transportation.
  • Current CPR certification is mandatory.

Desired Licenses

  • Palliative Care ARNP certification is preferred.
