Palliative Care Nurse Practitioner

We are seeking a dedicated Nurse Practitioner to join our hospice care team.
This is a PRN/Inconsistent role focused on Home Visits for Hospice patients.

Key Responsibilities
The Hospice Nurse Practitioner delivers expert consultation in palliative care, symptom management, and supportive services tailored to the needs of hospice patients as directed by the referring physician.
- Conducts in-person visits within designated timeframes to assess qualifying hospice patients and communicates clinical findings to the Hospice Medical Director.
- Collaborates with the Hospice Medical Director as necessary, informs the primary physician of recommended services, and works alongside other healthcare providers as needed.
- Develops and coordinates the care plan, documenting issues, objectives, interventions, and patient/family responses to hospice services. Engages with the patient/family, attending physician, and interdisciplinary team members to ensure comprehensive care.
- Educates and guides the patient/family in self-care practices when appropriate.
- Maintains precise and relevant clinical documentation regarding the patient's status. Communicates patient conditions and any identified needs or concerns to the Administrator and Hospice Medical Director.
Qualifications
Experience Requirements
- A minimum of five years of nursing experience; clinical expertise in oncology, hospice, geriatrics, or intensive care is preferred.
License Requirements
- Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
- Valid certification from a recognized national body such as ANCC, NBCHPN, or AANP.
- A valid driver's license and vehicle insurance, with access to reliable transportation.
- Current CPR certification is mandatory.
Preferred Licenses
- Palliative Care ARNP certification is preferred.
Skill Requirements
- Strong understanding of the interdisciplinary team approach in healthcare.
