Hospice Nurse Practitioner Position

We are seeking a dedicated Nurse Practitioner to join our hospice team.
This is a PRN/Inconsistent role focused on Home Visits for Hospice patients.

Role Responsibilities
The Hospice Nurse Practitioner offers expert consultation in palliative care, symptom management, and supportive services tailored to the needs of hospice patients as referred by their physicians.
- Conducts in-person visits to qualifying hospice patients within designated timeframes and communicates clinical findings to the Hospice Medical Director.
- Collaborates with the Hospice Medical Director as necessary, informs the primary physician of recommended services, and engages with other healthcare providers as needed.
- Develops and coordinates individualized care plans, documenting issues, goals, interventions, and patient/family responses to hospice services.
- Educates and guides patients and families in self-care techniques when appropriate.
- Keeps precise and relevant clinical records regarding patient conditions, reporting any concerns to the Administrator and Hospice Medical Director.
Experience Requirements
- A minimum of five years of nursing experience, with a preference for clinical nursing in oncology, hospice, geriatrics, or intensive care settings.
Licensure Requirements
- Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice.
- Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
- A valid driver's license and vehicle insurance, along with access to reliable transportation.
- Current CPR certification is mandatory.
Preferred Licensure
- Palliative Care ARNP certification is preferred.
Skill Requirements
- Strong understanding of the interdisciplinary team approach in healthcare.
