Job Summary

We are seeking a highly skilled and experienced Nurse Leader/Supervisor to join our team at RiverWoods Exeter. The successful candidate will be responsible for providing leadership and direction to our nursing staff, ensuring the delivery of high-quality patient care and promoting a positive and supportive work environment.

Key Responsibilities
  • Manage and coordinate the care of residents, utilizing the nursing process to ensure individualized care plans are developed and implemented.
  • Collaborate with team members to formulate and update resident care plans, ensuring that all aspects of care are addressed.
  • Assess and document changes in residents' physical or mental status, reporting findings to the primary care physician and treatment team.
  • Provide leadership and direction to direct care partners, preparing daily assignments and delegating workload as needed.
  • Supervise direct care team members, observing and evaluating their performance to ensure high-quality care is delivered.
  • Establish positive working relationships with team members, residents, and family members to facilitate open and direct communication.
  • Obtain, administer, and document medications and treatments given to residents, following professional guidelines and nursing department policies.
  • Inventory and maintain locked systems for narcotics, ensuring compliance with facility policies and procedures.
  • Cooperate with the overall needs of the facility during times of increased demand, completing tasks as assigned.
  • Complete nurses' notes and other required documentation in a timely manner, including monthly summaries, fall investigations, wound and skin assessments, etc.
  • Participate in Resident Care Plan Meetings and share responsibility for being on call for in-house staff, assuming coverage as team nurse when needed.
Requirements
  • Must possess a Nursing Degree or Diploma from an accredited college, university, or nursing school.
  • Long-term care facility experience and specialized knowledge of nursing care of the elderly and rehabilitative nursing are preferred.
  • Must hold a nursing license within the State of New Hampshire and maintain CEU's annually.
  • Must maintain CPR certification and be Resident Assessment Tool certified.
