This position is responsible for ensuring the accuracy and completeness of a patient's medication regimen upon admission, while in the Emergency Department or prior to planned surgery. The incumbent will work closely with the patient, family, pharmacies, and other healthcare professionals to obtain a comprehensive list of medications, including prescription and over-the-counter products, as well as supplements.
Key Responsibilities- Interview patients and utilize other resources to obtain a list of home medications, over-the-counter products, and supplements.
- Enter medication orders into the pharmacy computer system and maintain and update the patient medication reconciliation profile.
- Refer therapeutic questions to the pharmacist and use good judgment when providing information to patients.
- Assist in the training of new technicians, pharmacy students, pharmacy practice residents, and nursing staff in medication reconciliation.
- Work varying shifts and days to meet the needs of the department, which operates 24 hours a day, 7 days a week.
- One year of retail or two years of hospital experience as a Pharmacy Technician is required.
- Experience in a customer service role is preferred.
- Registered by the Nevada State Board of Pharmacy and nationally certified (CPhT) is preferred.
