Compassionate Community Care (CCC) has been a leader in home care and geriatric management in San Francisco since 2001. We specialize in tailored consulting, planning, and non-medical home care services for families with elderly clients, ensuring they can live comfortably and independently in their own homes.
Our team consists of both licensed and non-licensed caregivers who conduct regular home visits to assist with daily living activities, medication management, and more.
We are looking for a motivated, compassionate, and detail-oriented nurse who excels in multitasking and thrives in a dynamic environment.
Key Responsibilities:
- Build and nurture relationships with referral sources to promote CCC services and develop a steady stream of new clients.
- Maintain a comprehensive marketing database to track the progress of referral relationships.
- Enhance CCC's online presence and drive sales growth.
- Create and distribute marketing and outreach materials.
- Organize and participate in educational and promotional events, including sponsorship and speaking opportunities.
- Conduct thorough assessments of clients and their home environments to develop tailored care plans.
- Guide clients through the intake process, clarifying paperwork and financial responsibilities.
- Address crises or urgent situations for clients and families as a Geriatric Care Manager (GCM) or Aging Life Care Manager (ALCM).
- Ensure that the needs and requests of clients, referrals, and colleagues are met effectively.
- Collaborate with team members and professionals to enhance the efficiency and growth of CCC.
- Participate in an on-call rotation, including weekends and holidays.
- Train caregivers on essential skills such as the use of hoyer lifts, gait belts, and proper handling of clients with dementia or Alzheimer’s.
- Assist with staffing and scheduling as needed.
- Provide timely updates to the Executive Director and team members.
- Understand and align with CCC's mission and objectives.
Qualifications:
- Demonstrated success in marketing and outreach is preferred.
- Current RN or LVN license, with a Bachelor's or Master's degree in Gerontology, healthcare, or a related field.
- Experience and knowledge in geriatric and elder care.
- Able to train caregivers in essential care techniques.
- Strong listening skills, flexibility, and a customer-oriented approach.
- Excellent communication skills, both written and verbal, in English.
- Proficient in computer applications including Word, Excel, Outlook, and social media.
- Strong problem-solving abilities.
- Self-motivated and capable of managing multiple tasks.
- Reliable and composed in challenging situations.
- Experience working with diverse populations.
- Positive and professional demeanor towards clients and staff.
- Able to work independently and collaboratively, with sound judgment and minimal supervision.
- Attention to detail and commitment to follow-through.
- Strong time management skills.
- Valid driver's license and reliable transportation preferred.
- Successful completion of pre-employment health and background checks is required.
Preferred Skills:
Experience in training caregivers to assist with daily living activities and manage clients with complex needs, including dementia and other behavioral challenges.
This is a full-time, exempt position with a competitive compensation and benefits package, including medical, dental, vision, life insurance, 401K options with company matching, paid vacation, and sick leave.

Pay: $75,000.00 – $100,000.00 per year
