Geriatric Care Specialist

We are seeking a skilled Nurse Practitioner to provide high-quality patient care and support to individuals in long-term care settings. As a Geriatrics Specialist, you will be responsible for conducting thorough assessments, developing individualized care plans, and collaborating with healthcare teams to ensure seamless patient care.
Key Responsibilities- Deliver comprehensive geriatric care to patients in long-term care facilities, focusing on their unique needs and circumstances.
- Conduct thorough assessments to identify patients' strengths, weaknesses, and care requirements, and develop individualized care plans to address their needs.
- Collaborate with healthcare teams, including physicians, therapists, and other healthcare professionals, to ensure that patients receive coordinated and effective care.
- Stay up-to-date with the latest geriatric care practices and guidelines, and apply this knowledge to improve patient outcomes.
- Active and unrestricted license in the state of practice.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions.
- Access to reliable transportation.
- Collaborative practice agreement, if applicable in the state of practice.
