JOB SUMMARY:
The Rehabilitation Specialist will assess all patients/residents admitted to the facility and will
provide written recommendations to the attending physician for a rehabilitation program if necessary. Upon
receipt of the physician's directives, the Rehabilitation Specialist will be accountable for the
execution of services and programs related to physical therapy.

The Rehabilitation Specialist
evaluates, documents, and performs assessments on patients/residents as required.
Participates actively in all departmental staff meetings. The Specialist will
aim to assist patients/residents in alleviating discomfort, enhancing mobility, maintaining functional
independence, and achieving optimal rehabilitation outcomes. Conducts and facilitates in-service and
orientation sessions as well as on-the-job training initiatives. The Rehabilitation Specialist will collaborate
with the interdisciplinary team through effective communication and teamwork to
ensure a safe discharge at the highest level of patient/resident function. The Rehabilitation Specialist
will comply with patient/resident rights, uphold professional ethics, and adhere to all applicable state and federal regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Demonstrates understanding of age-specific developmental factors pertinent to adult and
geriatric residents (i.e., physical, cognitive, and social factors) in planning and
delivering care.

  • Exhibits knowledge, skills, and techniques necessary to care for and treat

residents with the following needs:
orthopedic conditions, neurological disorders, infectious diseases, and
cognitive impairments, as applicable.

  • Evaluates all patients/residents admitted to the facility within 48 hours.
  • Provides written recommendations to the attending physician for a rehabilitation program, if necessary.
  • Upon receipt of physician's directives, implements services and programs related to physical
therapy.

  • Conducts patient/resident evaluations, establishes short and long-term Physical Therapy
goals, assesses rehabilitation potential, and formulates and executes a treatment plan
which may be delegated to a Physical Therapy Assistant if applicable.

  • Supervises Physical Therapy Assistants in accordance with relevant regulations.
  • Engages in professional communication.
  • Completes all necessary documentation in patient/resident records and care plans.
  • Ensures that appropriate equipment is ordered for home discharge and provides a Home Exercise
Program as needed.

  • Offers suitable patient/resident and family education.
  • Consistently demonstrates dependability and
punctuality.

  • Maintains a neat, professional appearance and attire.

    • Performs any other necessary duties as assigned by Supervisor.

REQUIRED SKILLS AND ABILITIES:

among colleagues. Interacts positively with residents, family members, and visitors in all
situations.

  • Exhibits excellent communication skills and professionalism.
  • Demonstrates ability to coordinate activities with other departments to
ensure patients/residents feel safe and empowered. Keeps family members informed about patient/resident progress in Therapy.

  • Strives for excellence by performing duties with accuracy and timeliness.
  • Displays the ability to problem-solve quickly and efficiently in a fast-paced environment.
  • Provides excellent customer service and support to all members of the
interdisciplinary team and assists visitors, staff, volunteers, and vendors as needed.

QUALIFICATIONS:

Education:
Masters or Doctorate of Physical Therapy Degree from an accredited institution

Experience:
Minimum: Newly licensed Physical Therapist
Preferred: 2-3 years of experience in a skilled nursing environment

Licensure:
Valid Physical Therapy License in the state of practice

PAY RATE:
$48-$50 per hour, based on experience
