United Hospice, Inc. is a not-for-profit organization dedicated to providing compassionate care and support to individuals and families facing serious illnesses in our community. We are currently in search of a Licensed Physical Therapist to join our team in a per diem capacity, contributing to our mission of delivering exceptional hospice services.
Compensation: The pay rate for this role is competitive, reaching up to $60.00 per hour, based on relevant experience.
Role Summary:The Physical Therapist (PT) is responsible for assessing patients, formulating effective treatment strategies, and collaborating with healthcare providers to set achievable patient objectives.
Key Responsibilities:- Collaborate with physicians to create tailored physical therapy plans based on thorough patient evaluations.
- Conduct initial assessments of patients' physical capabilities, followed by ongoing evaluations as directed by medical orders.
- Identify patient needs and develop individualized therapy care plans, making necessary adjustments and discussing these plans with the care team.
- Administer treatments aimed at alleviating pain, enhancing or restoring function, and promoting optimal performance.
- Guide patients through both active and passive range of motion exercises, muscle re-education, and daily living activities, including transfer techniques.
- Monitor, document, and communicate patients' responses to treatment and any notable changes in their condition to the patient, family, and physician.
- Provide education to caregivers regarding treatment plans and assess their capability to assist patients effectively.
- Engage in team meetings and contribute to collaborative care efforts.
- Participate in quality assurance and performance improvement initiatives, as well as in-service training provided by the hospice.
- Assume additional responsibilities as assigned.
- Possession of a valid New York State license in Physical Therapy is mandatory.
- A minimum of one year of experience in the field of physical therapy is required.
- Strong oral and written communication skills are essential.
- Demonstrated ability to solve problems effectively.
- Capability to engage with a diverse population in a friendly and effective manner.
- Commitment to the principles of hospice care.
- Proficiency in Microsoft Office, electronic medical records, and the ability to learn new software systems.
- Access to a vehicle, a valid driver's license, and current insurance are required.
