Position Overview

Key Responsibilities:

· Upon receiving a new client assignment, the Outreach Services Coordinator is responsible for promptly reaching out to the client and scheduling a meeting at a designated office or alternative location to finalize necessary documentation.

· For each assigned case:

· Establish client objectives and align support activities accordingly.

o Monitor the advancement of these objectives and modify care plans as necessary.

o Accurately and timely collect and input data and notes, documenting actions and results into case management systems.

o Strategically plan and implement care activities to ensure client objectives are met while minimizing potential billing and reimbursement issues for the organization.

o Efficiently manage time to achieve required outcomes for clients within the timeframe established by the Supervisor.

o Maintain proactive communication with the Outreach Supervisor regarding any emerging challenges and necessary adjustments.

o Engage with each assigned client at least four times per month.

o Review the assigned caseload with the Outreach Supervisor bi-monthly to ensure effective case management and make adjustments as needed.

o Build and sustain productive relationships with key individuals at external organizations.

o Attend relevant meetings at the Department of Community Mental Health to discuss client cases as required.

o Conduct street outreach assessments (VI-SPDAT) as directed by the Outreach Supervisor.

o Utilize one's ongoing experience and training to demonstrate an evolving ability to problem-solve, advocate, mediate, and manage increasingly complex tasks related to care coordination.

o Challenge the status quo and strive for excellence in achieving the necessary outcomes for clients and the organization.

o Perform additional tasks as assigned.

· For all field activities:

o Conduct one-on-one meetings with individuals seeking services to assess eligibility.

o Complete 3-5 intake assessments during each shelter visit, totaling 10-15 intakes weekly.

o Proactively communicate with the Outreach Supervisor regarding trends, relationships, and developments observed in the field.

o Monitor and document activities and outcomes.
