Job Title: Case Management Supervisor

Join Santa Clara Family Health Plan as a Case Management Supervisor and take on a challenging role that requires strong leadership and case management skills. As a key member of our Health Services team, you will be responsible for overseeing daily operations, developing and implementing new programs, and coordinating case management referrals.

Key Responsibilities:
  • Develop and implement case management workflows in compliance with regulatory requirements and SCFHP policies.
  • Monitor and improve quality of case management processes, including timely completion of Health Risk Assessments and Individual Care Plans.
  • Collaborate with the Manager of Case Management on audits and corrective action plans.
  • Ensure integration of Case Management operations with other internal and external teams.
  • Resolve complex issues and escalate to management as needed.
  • Establish effective relationships with staff, members, providers, and external partners.
Supervisory Responsibilities:
  • Recruit, interview, and hire staff.
  • Develop a high-performing department culture and motivate employees.
  • Train and mentor staff, and initiate retraining as needed.
  • Set goals, assign work, and direct staff to achieve objectives.
  • Appraise performance, reward, and discipline staff as needed.
Requirements:
  • Unrestricted professional clinical licensure in California.
  • Minimum two years of experience in a supervisory or lead capacity in case management or utilization management.
  • Current working knowledge of Medicare and MediCal regulations and NCQA Population Health standards.
  • CMSA Certification in Case Management (desired).
  • Valid California driver's license and acceptable driving record (or ability to use alternative transportation).
  • Spanish, Vietnamese, Chinese, or Tagalog language bi-lingual skills (desired).
Physical Requirements:
  • Mobility: regular bending, reaching, and sitting/standing for extended periods.
  • Lifting: regularly lift and carry files and supplies up to 5 pounds.
  • Visual: ability to read information and perform close-up work.
  • Dexterity: regular use of hands, wrists, and finger movements.
  • Hearing/Talking: ability to hear and talk to exchange information.
