Social Worker in Hospice Care

We are seeking a compassionate Social Worker to join our hospice care team in Maine. As a Hospice Social Worker, you will play a vital role in providing holistic care to patients and their families. Job Description:
The Hospice Social Worker will assess patients' needs, develop care plans, and provide support to empower patients and families to achieve maximum wellness and independence. This role requires a strong educational background, with a Master's degree in Social Work, and licensure as an LCSW or LMSW-CC in the state of Maine.
Responsibilities:
- Conduct home visits to assess patients' social, emotional, and environmental needs
- Develop and implement individualized care plans that address patients' needs and goals
- Provide education and support to patients and their families on managing chronic conditions and accessing community resources
- Collaborate with interdisciplinary teams to ensure comprehensive care coordination
Requirements:
- Master's degree in Social Work from an accredited program
- Current licensure as an LCSW or LMSW-CC in the state of Maine
- Basic Life Support certification for Healthcare Providers
- Strong communication and interpersonal skills
Salary and Benefits:
Our competitive salary range for this position is $65,000 - $85,000 per year, based on experience.
