Hospice Social Work Specialist

As a Hospice Social Worker, you will have the opportunity to work with patients and families in their homes, providing skilled, cost-effective social work care that empowers individuals to achieve maximum levels of wellness and independence.
About the Position:
- 40 hours/week, day shift
- Maine state licensure as an LMSW or LCSW required
- Valid driver's license and liability insurance
- BLS Certification (obtainable within 30 days)
Key Responsibilities:
- Assess patient needs and develop personalized care plans
- Collaborate with healthcare teams to provide holistic care
- Provide education and support to patients and families
Salary Information:
We offer a competitive salary range of $55,000 - $65,000 per year, depending on experience.
