Job Summary:

The Community Health Center of Buffalo, Inc. is seeking a skilled Mental Health Counselor to join our team. As a Mental Health Counselor, you will be responsible for providing case management and counseling services to clients with bio-psychosocial needs. This role involves assessing clients' needs, providing short-term counseling, crisis intervention, and coordinating support services.

Responsibilities:
  • Conduct comprehensive psychosocial assessments to determine treatment and referral needs for clients.
  • Assist clients in accessing entitlements, insurance, and community resources to reduce reliance on emergency assistance programs.
  • Advocate for clients when engaging with community agencies and organizations.
  • Maintain accurate client records, prepare reports, and documentation as necessary.
  • Coordinate transportation assistance for clients as needed.
  • Attend clinical and team meetings, participate in case conferences.
  • Identify educational and support service needs for clients, such as pregnancy counseling, parenting skills, and caregiver support.
  • Conduct need assessments and education for specific client populations.
  • Collaborate with healthcare providers and make referrals as required.
  • Provide counseling and testing for HIV and other health-related services.
  • Follow up on missed appointments and ensure continuity of care for clients.
  • Contribute to Quality Improvement and Quality Assurance activities.
  • Maintain a clean, safe, and organized work environment.
  • Attend mandatory meetings and fulfill job-related duties as assigned.
Qualifications:
  • Licensed Mental Health Counselor (LMHC) or NYS Permit holder.
  • Minimum of three (3) years of experience as a Mental Health Counselor, with excellent interpersonal and communication skills.
  • Preferred experience with trauma-informed care and EMDR Therapy.
  • Proficiency in MS Word, Excel, and Outlook.
  • Ability to manage physical demands, including sitting, standing, walking, bending, and occasional weekend and evening work requirements.
Salary Range:

$53,000 - $60,000 annually
