About the Role

CommonSpirit Health is seeking a dedicated and compassionate Hospice Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families during the end-of-life journey.

Key Responsibilities
  • Assess and develop individualized care plans for patients and their families
  • Provide skilled nursing care and treatments in the home or community setting
  • Collaborate with interdisciplinary teams to coordinate care and services
  • Document patient progress and outcomes
  • Communicate effectively with patients, families, and healthcare teams
Requirements
  • Current RN license issued by the State of Oregon
  • Two years of acute care nursing experience
  • One year of hospice nursing experience or two years of home health nursing experience preferred
  • Associate's Degree in Nursing required
