Job Summary

The Hospice RN plays a vital role in providing client care in the home, place of residence, or community site. This position requires a broad spectrum of knowledge in nursing to promote an optimal level of client and family well-being.

Key Responsibilities
  • Assess and develop a plan of care through interviews with patients and primary caregivers/family, incorporating physical assessment and previous health and psychosocial history.
  • Provide case management and document skilled care, treatments, and diagnostic procedures.
  • Complete and document initial evaluations, assess patient status, and inform physicians and supervisors of changes in patient condition.
  • Make referrals as appropriate and authorized by physicians, coordinate services through consultation and interdisciplinary conferences.
Requirements
  • Current RN license issued by the State of Oregon.
  • American Heart Association BLS for Health Care Providers within 30 days of hire.
  • Valid Oregon Driver's License, automobile insurance, and provide own transportation.
  • Two years of acute care nursing experience required; one year of hospice nursing experience or two years of home health nursing experience preferred.
  • Associate's Degree in Nursing required.
