CommonSpirit Health is seeking a dedicated and compassionate Hospice Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities- Assess and develop individualized care plans for patients, incorporating their unique needs and goals.
- Provide skilled nursing care, including wound care, medication management, and symptom control.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and their families.
- Document patient progress, including assessments, care plans, and outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current RN license issued by the State of Oregon.
- Two (2) years acute care nursing experience required.
- One (1) year hospice nursing experience or two (2) years home health nursing experience preferred.
- Associate's Degree in Nursing required.
