Job Summary

We are seeking a highly skilled RN Care Manager to join our team at UNC Health. As a key member of our care management team, you will be responsible for providing comprehensive support and expertise to patients and their families. Your primary goal will be to enhance the quality of patient management and satisfaction, while promoting continuity of care and cost-effectiveness.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized plans of care to meet the unique needs of each patient.
  • Collaborate with multidisciplinary teams to identify and address barriers to discharge, ensuring timely and effective transitions of care.
  • Provide education and support to patients and families, empowering them to take an active role in their care.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in care management.
Requirements
  • Graduation from a state-accredited school of professional nursing.
  • Current licensure as a Registered Nurse in the state of North Carolina.
  • Two years of healthcare experience as a Registered Nurse.
  • Strong assessment and critical thinking skills, with the ability to prioritize and manage multiple patients and tasks.
