Job Summary:

The RN Care Manager will be responsible for assessing patient needs, developing and implementing care plans, and coordinating with healthcare teams to ensure seamless transitions of care. This is a contract on-site role for a RN Care Manager in Chapel Hill, NC.

Key Responsibilities:
  • Conduct comprehensive assessments of patients to identify their needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure continuity of care and effective communication.
  • Coordinate with social workers, utilization managers, and other healthcare professionals to address patient needs.
  • Participate in daily care management touchpoints and CAPP meetings to review patient progress and identify areas for improvement.
  • Modify discharge plans based on information shared at CAPP meetings and collaborate with the care team to ensure successful transitions of care.
  • Identify patients with barriers to discharge and develop strategies to address these challenges.
  • Provide education on community resources, support groups, and other relevant information to patients and their families.
  • Coordinate referrals to post-acute providers and community resources as needed.
Requirements:
  • Two (2) years of nursing experience.
  • ADN or ASN from an accredited school of nursing.
  • BSN from an accredited school of nursing.
  • Licensed to practice as a Registered Nurse in the state of North Carolina.
  • Strong assessment and critical thinking skills.
