Job Summary

We are seeking a skilled Physical Therapist to join our team at Yale New Haven Health. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes, under the direction of the attending physician.

Key Responsibilities
  • Conduct comprehensive evaluations of patients' functional abilities and develop individualized therapy plans.
  • Participate in case management conferences and maintain accurate documentation of patient progress.
  • Provide instruction and supervision to patients, caregivers, and other clinical staff on therapy plans and techniques.
  • Select and administer diagnostic and therapeutic techniques and materials as needed.
  • Supervise Physical Therapy Assistants and Home Health Aides, ensuring they are adequately trained and equipped to provide quality care.
  • Ensure timely and accurate documentation of patient visits and progress in the Electronic Medical Record (EMR).
  • Collaborate with patients and caregivers to obtain necessary equipment and provide instruction on its use.
  • Monitor productivity and communicate needs for additional visits or resources as necessary.
  • Verify insurance authorization prior to visits when required.
  • Report incidents and concerns to the RN case manager and Manager, Therapy Services.
  • Participate in staff meetings, in-service programs, and other meetings as requested.
Requirements
  • Bachelor of Science in Physical Therapy required; master's or doctorate desirable.
  • Minimum three years of clinical experience in Physical Therapy, preferably in an acute or sub-acute care setting; home care experience is a plus.
  • Valid Physical Therapist license in Connecticut or eligibility for licensure.
  • Excellent written and verbal communication skills, with ability to work collaboratively with multidisciplinary teams.
  • Strong computer skills and adaptability to learn quickly.
