Physical Therapist Home Care Specialist

We are seeking a skilled Physical Therapist to join our team at LHC Group, a leading provider of post-acute care services. As a Physical Therapist, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes.
About the RoleThe successful candidate will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing interventions to improve patient outcomes. You will work closely with our interdisciplinary team to ensure seamless care coordination and excellent patient satisfaction.
Responsibilities- Conduct thorough assessments and evaluations of patient care needs
- Develop and implement individualized treatment plans to address patient goals and objectives
- Provide skilled interventions to improve patient mobility, function, and overall well-being
- Collaborate with the healthcare team to ensure comprehensive care coordination
- Document patient progress and outcomes in accordance with regulatory requirements
- Current licensure to practice physical therapy in the state of Connecticut
- Current CPR certification
- Access to a dependable vehicle or public transportation
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
