POSITION OVERVIEW

The Prep Academy Case Manager is responsible for overseeing the delivery of services to children and families, assisting them before their enrollment in the Prep Academy and supporting them after their transition out of the program. Daily responsibilities will primarily involve planning and coordinating care during the transitional phases associated with the Prep Academy Program. This role also entails managing billing for Community Psychiatric Supportive Treatment (CPST) services through insurance.

KEY RESPONSIBILITIES
  • Foster a welcoming and inclusive atmosphere for clients, colleagues, and community partners from various backgrounds and experiences.
  • Incorporate Trauma-Informed Care principles in all aspects of service delivery.
  • Demonstrate sensitivity to the cultural and socioeconomic backgrounds of children to provide optimal care.
  • Ensure a safe and structured environment that is free from any form of abuse.
  • Serve as a role model for children and staff, promoting appropriate interactions and coping strategies.
  • Establish strong relationships with clients, understanding the importance of responding with empathy, sensitivity, and respect.
  • Effectively de-escalate situations while maintaining the dignity and integrity of all involved.
  • Supervise a group of children with Severe Emotional Disturbances (SED) effectively.
  • Build strong partnerships with families to facilitate necessary services.
  • Collaborate with youth case workers, clinicians, nurses, and other program supervisors to ensure seamless service coordination.
  • Provide crisis intervention as needed.
  • Maintain accurate and ongoing documentation for each child's record in compliance with agency and regulatory standards, including medical history, consent forms, and CPST notes.
  • Deliver community psychiatric support and treatment (CPST) to clients regularly.
  • Assist clinicians with the discharge process, including communicating follow-up appointment details.
  • Perform other assigned duties as required.
  • Work schedule will be flexible to accommodate the needs of the families served, including evening hours.
QUALIFICATIONS
  • A minimum of three (3) years of experience working with children and families with special needs.
  • A bachelor's degree in social services, psychology, or a related field is preferred.
  • Basic understanding of child development is essential.
  • Must possess a valid driver's license, valid automobile insurance, and valid vehicle registration.
  • Must be insurable by the St. Vincent Family Services insurance provider.
COMMUNICATION SKILLS: Exceptional verbal and written communication skills are essential.

Ability to engage with diverse audiences, both internal and external to the organization.

MATHEMATICAL SKILLS: Basic mathematical skills are required.

TECHNICAL SKILLS: Proficiency in Microsoft Office applications and the ability to interpret complex regulatory documents.

REASONING ABILITY: Ability to manage multiple tasks and resolve complex interpersonal issues.
