ROLE SUMMARY

The Housing Support Specialist will deliver comprehensive, community-oriented case management services to individuals and families transitioning from homelessness. This role involves utilizing established best practices to conduct intensive case management, perform assessments, develop personalized service plans, connect clients to essential resources, and provide crisis intervention for clients facing various challenges, including medical, mental health, and substance use issues. This position requires collaboration with local organizations and service providers to ensure clients receive tailored support that addresses their housing, health, and income needs.

KEY RESPONSIBILITIES

  • Build trusting relationships with clients transitioning from homelessness using Trauma-Informed Care, Harm Reduction, and Housing First methodologies.
  • Deliver intensive case management services that encompass independent living skills, housing stabilization, financial management, community integration, employment assistance, and access to health services.
  • Collaborate with clients to conduct thorough biopsychosocial assessments and create individualized goal plans, reviewing and updating them regularly.
  • Maintain accurate client records in compliance with agency policies and documentation standards.
  • Provide crisis prevention and intervention strategies, including safety planning and de-escalation techniques.
  • Advocate for clients to resolve issues with landlords and access eviction prevention resources.
  • Engage in ongoing professional development and training to stay informed about best practices in housing support and homelessness prevention.
  • Participate in case conferences, team meetings, and community events as required.
  • Complete additional tasks and responsibilities as assigned by the supervisor.

ADDITIONAL DUTIES:

  • Provide field-based support services, requiring travel to various locations to assist clients.
  • Connect clients with additional resources, including supportive housing options.

SKILLS AND QUALIFICATIONS

  • Understanding of the challenges faced by individuals experiencing homelessness, including health, substance abuse, and mental health issues.
  • Familiarity with best practices in homeless services, such as Housing First and Trauma-Informed Care.
  • Ability to engage empathetically with clients from diverse backgrounds.
  • Strong advocacy and conflict resolution skills.
  • Proficient in electronic data management and documentation.
  • Excellent communication skills, both written and verbal, with the ability to work independently and collaboratively.
  • Bilingual proficiency in Spanish is a plus.

WORK ENVIRONMENT AND REQUIREMENTS

  • Flexibility to work a varied schedule, including evenings and weekends as needed.
  • Ability to travel frequently within the service area.
  • Maintain confidentiality in accordance with HIPAA standards.
