The Union West Transition Services clinic is an outpatient facility aimed at assisting patients in regaining their health and well-being. This multidisciplinary clinic caters to individuals at elevated risk for complications and hospital readmissions due to complex medical conditions or barriers to accessing healthcare. Our approach includes integrated consultations with healthcare providers, pharmacists, and social workers. During a comprehensive 30-day follow-up period, we focus on identifying and overcoming health barriers while promoting patient engagement, access, and education. Care is provided in-office, virtually, or through community paramedicine, tailored to the needs of each patient. This service is complemented by a traditional Internal Medicine clinic, ensuring exceptional clinical care and a patient-centered experience.
Key Features of the Role:
- 100% Outpatient Care
- Collaborative environment with a team of 1 Physician and 1 Advanced Practice Provider
- Flexible office hours from Monday to Friday (7 AM - 5 PM)
- Shared call rotation, phone coverage only
- No Night or Weekend Shifts
Ideal Candidate Profile:
- Board-Eligible or Board-Certified in Internal Medicine
- Interest in treating adult patients aged 18 and older
- Commitment to delivering high-quality patient care in a collaborative and productive clinic setting
