Job Overview

UPMC Home Health is seeking a dedicated Physical Therapist to join our team, providing services across various communities. This role primarily operates during daylight hours, Monday through Friday, with the flexibility to start from home and travel to patient locations.
As a Physical Therapist, you will deliver professional therapy services tailored to individuals of all ages in both home and community environments. Your focus will be on assessing, intervening, treating, and instructing patients to enhance their functional capabilities and ensure they receive the highest quality of care.

Key Responsibilities:

  • Prioritize customer satisfaction by delivering exceptional service and care.
  • Engage in continuous professional development through ongoing education and training.
  • Conduct thorough evaluations, develop treatment plans, and implement effective physical therapy interventions, utilizing appropriate modalities and assistive devices.
  • Identify and participate in quality improvement initiatives within the agency's Home Health or Early Intervention programs.
  • Supervise and guide Certified Physical Therapy Assistants as necessary.
  • Complete compliance training and adhere to organizational standards and policies.
  • Maintain professionalism in all interactions with patients, families, colleagues, and other healthcare professionals.
  • Ensure accurate and timely documentation of all patient care activities, adhering to agency and regulatory standards.
  • Collaborate with interdisciplinary team members, families, and physicians to optimize patient outcomes.
  • Develop and implement individualized physical therapy treatment plans in coordination with referring physicians, focusing on maximizing patient independence.

Qualifications include graduation from an accredited Physical Therapy program recognized by relevant professional associations, eligibility for the National Registration Examination, or equivalent experience. A minimum of one year of experience is required for independent practice, with less than one year acceptable under supervision.

Licensure and Certifications:
Candidates must possess the following certifications:

  • Automotive Insurance
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR)
  • Valid Driver's License
  • Licensed Physical Therapist (PT)
