Registered Nurse for Hospice Care

UPMC Family Hospice is seeking a dedicated Registered Nurse for the hospice care team.

In this role, you will have the unique opportunity to build meaningful relationships with patients and their families, guiding their care in the familiar surroundings of their homes.
Work Schedule: Full-time, Monday through Friday, with daylight shifts and rotations for on-call and weekend duties.
As a Registered Nurse at UPMC Family Hospice, you will collaborate with a multidisciplinary team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to deliver tailored patient care.

Responsibilities:
- As the nurse, you will develop patients' care plans in collaboration with physicians and other healthcare professionals to meet their needs.
*Coordinate case management activities across a multidisciplinary healthcare team to ensure high-quality patient care.
- Initiate, deliver, and assess patient- and family-centered health education.
- Strive to help patients achieve optimal health outcomes, providing compassionate care focused on the patient's experience.
*Complete all necessary documentation, including visit assessments and verbal orders, accurately and promptly while adhering to regulatory standards.
*Participate in compliance training and follow the organization's standards of conduct, policies, and procedures, along with all local, state, and federal regulations.
Qualifications:
- Graduation from an accredited nursing program is required.
- A Bachelor's degree is preferred.
- Experience in an Acute Care Facility or Skilled Care Facility is preferred.
Licensure and Certifications:
- Current licensure as a Registered Professional Nurse in the state of practice or in a state covered by a licensure compact agreement.
- CPR certification is required based on AHA standards, including both a didactic and skills demonstration component.
- Automotive Insurance, Driver's License, Act 33, Act 34, and OAPSA clearances are required.
