Job Summary

We are seeking a skilled Assessment Nurse Specialist to join our team at HealthCare Support in White Plains, NY.

About the Role

This is a full-time position, 100% field-based, with Monday-Friday, 8:30 am - 5:30 pm schedule. The successful candidate will be responsible for evaluating patients' physical, mental, and social conditions through in-person or telehealth interviews. Accurate documentation of assessment findings in the UAS-NY system is essential, adhering to state regulations and organizational policies.

Key Responsibilities
  • Evaluate patients' physical, mental, and social conditions through in-person or telehealth interviews.
  • Document assessment findings accurately in the UAS-NY system in compliance with state regulations and organizational policies.
Requirements

To be considered for this role, you should have:

  • A Bachelor's degree in Nursing or a related health field (preferably).
  • An active and unrestricted New York State RN license.
  • At least 2 years of experience performing UAS assessments.
  • 3+ years of experience in Managed Long Term Care (MLTC), Long-Term Home Health Care Program (LTHHCP), Medicare Advantage Plan (MAP), PACE, or other managed care programs.
  • 5+ years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning.
Compensation and Benefits

The salary range for this position is $110,000 - $115,000 per annum, plus an annual bonus. Our benefits package includes medical, dental, and vision coverage, a 401(k) plan with matching contributions, pension, short-term and long-term disability benefits, and 28 days of paid time off plus 9 paid holidays.
