Salary and Benefits

The salary for this position is $103,000 - $129,000 per year, plus a sign-on bonus of up to $15,000 paid out within the first year. Our comprehensive benefits package includes accrued vacation days, sick days, holidays, and free parking. Additionally, we offer participation in a 403(b) retirement plan, short-term disability insurance, life insurance, cancer insurance, flex spending, and transit check.

Job Description

We are looking for a seasoned nurse with experience in medical-surgical, community health, and/or hospice, palliative care, oncology nursing who can effectively communicate with patients and families regarding available hospice services. The successful candidate will possess strong interpersonal skills and be able to develop relationships with hospitals, physicians, nursing facilities, and other referral sources through outreach, education, and networking.

  • The Senior Hospice Admissions Nurse Specialist will be responsible for visiting patients who have been referred for admission to the Hospice at Home Program and informing them of the available services. They will also assess patients for eligibility and work with the interdisciplinary team to develop an initial plan of care utilizing principles of hospice and palliative care.
  • Responsibilities include generating and responding to referrals from hospitals and other community partners, working closely with physicians and the interdisciplinary team, and traveling on weekends as needed.
Required Skills and Qualifications

To be successful in this role, candidates must possess a current nursing license to practice in the state where they will be providing service, a valid driver's license, and possession of a registered and insured motor vehicle. Two years of experience working in an acute care setting are required, with one year of hospice experience preferred. A graduate degree from an accredited school of nursing, preferably with a BSN, is also preferred.
