LPN / Hospice Nurse

At Haven Hospice, we are dedicated to providing high-quality care to our patients and their families. As a Hospice LPN, you will play a vital role in our team, supporting patients with direct care and coordinating with our interdisciplinary team to ensure an optimal care experience.
Responsibilities- Provide direct patient care, including pain and symptom management
- Implement nursing practice based on comprehensive assessments and plans of care
- Document patient services accurately and timely
- Assess and educate patients and their families/caregivers
- Participate in coordination of hospice services
- Manage pain and symptoms effectively, evaluating patient response to treatments/medications
- Participate in professional growth and development opportunities, as well as quality assessment activities
- Ensure compliance with state and federal legal and regulatory requirements
- Graduate of an accredited practical or vocational nursing program, with current licensure to practice in the state of operation
- 1 year of nursing experience, with 3+ years preferred
- Experience in community health, hospice, or med/surg preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
