Hospice LPN Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Haven Hospice. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities:
- Provide direct patient care, including pain and symptom management
- Implement nursing practice following a comprehensive assessment and plan of care
- Document patient services accurately and timely
- Assess and provide patient and family/caregiver education
- Participate in coordination of hospice services
Requirements:
- Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation
- 1 year nursing experience required, 3+ years preferred
- Experience in community health, hospice or med/surg preferred
