Job Title: LPN / Licensed Practical Nurse - Hospice

Job Summary:

We are seeking a skilled and compassionate LPN to join our Hospice team. As an LPN, you will provide direct patient care, coordinate with the interdisciplinary team, and ensure that our patients and their families receive the best possible care experience.

Responsibilities:
  • Provide direct patient care, including pain and symptom management
  • Implement nursing practice based on comprehensive assessments and plans of care
  • Document patient services accurately and in a timely manner
  • Assess and educate patients and their families/caregivers
  • Participate in coordination of hospice services
  • Manage pain and symptoms effectively, evaluating patient response to treatments/medications
  • Participate in professional growth and development opportunities, as well as quality assessment activities
  • Ensure compliance with state and federal regulations
Qualifications:
  • Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation
  • 1 year of nursing experience required, 3+ years preferred
  • Experience in community health, hospice, or med/surg preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
