About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice care team. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in their time of need.

Responsibilities
  • Provide direct patient care, including pain and symptom management, and implement nursing practices based on comprehensive assessments and care plans.
  • Document patient services accurately and timely, and assess and provide patient and family/caregiver education.
  • Participate in the coordination of hospice services and provide appropriate pain and symptom management, evaluating patient response to treatments and medications.
  • Ensure compliance with state and federal legal and regulatory requirements, and participate in professional growth and development opportunities and quality assessment activities.
Requirements
  • Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation.
  • 1 year of nursing experience required, with 3+ years preferred.
  • Experience in community health, hospice, or med/surg preferred.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
