Illumination Foundation (IF) is a dynamic non-profit organization committed to breaking the cycle of homelessness through targeted, interdisciplinary support in our recuperative care centers, emergency shelters, housing services, and programs for children and families. Currently, IF operates over 13 facilities with more than 22 micro-communities across Orange County, Los Angeles County, and the Inland Empire.

Position Summary

The primary objective of the CSS Housing Navigation role is to deliver services to individuals with chronic health and/or mental health conditions who have substantial healthcare needs and/or are facing homelessness. The Lead Housing Case Manager engages directly with program participants, developing, arranging, and coordinating treatment services. This role aims to enhance the health of homeless individuals by facilitating stable housing, crafting comprehensive treatment plans, and linking clients to medical care, mental health services, substance use treatment, and other supportive resources. After securing permanent housing, the Lead Housing Navigator will assist clients in sustaining their housing. This position necessitates a commitment to confidentiality and the ability to manage sensitive information with discretion.

The compensation for this role is $28.00 per hour, based on experience.

Key Responsibilities

Leadership Duties:

  • Collaborate with management to address barriers in staff and client care during team discussions.
  • Engage with management to review team operations and program developments.
  • Participate in ICT meetings.
  • Train Housing Navigators, fostering teamwork, growth, efficiency, and effectiveness.
  • Assist in the training and resource provision for Housing Navigators.
  • Work with management to assess the strengths and weaknesses of CalAIM policies and programs.
  • Monitor and evaluate Housing Navigator productivity.
  • Coordinate regular check-ins with assigned Housing Navigators.
  • Gain familiarity with quality assurance processes, including chart audits.
  • Understand program and contract policies, especially regarding deliverables and structures.
  • Participate in onboarding processes, leveraging housing expertise to enhance recruitment.
  • Oversee Housing Navigator activities both in and out of the office.
  • Maintain ongoing communication with Program Coordinators and Housing Managers.

Housing Responsibilities:

  • Create personalized treatment plans with measurable goals, assisting participants in achieving their desired outcomes.
  • Network and collaborate with housing providers, landlords, and property managers to expand housing options for the program, including temporary and permanent solutions.
  • Advocate for clients to ensure they receive maximum benefits and resources, supporting their journey towards self-sufficiency.
  • Build relationships with representatives from Illumination and other agencies to facilitate access to services such as housing, mental health care, medical treatment, financial assistance, and legal advocacy.
  • Work alongside Housing Navigators to establish housing goal plans and make referrals to appropriate resources.
  • Conduct housing inspections to comply with HUD Housing Habitability standards.
  • Explore innovative housing solutions for client needs.
  • Attend relevant subcommittee and implementation meetings.
  • Coordinate, prepare, and maintain required documentation in a timely and thorough manner.
  • Engage in training and development opportunities as deemed appropriate by the supervisor.
  • Maintain client records in accordance with HIPAA regulations and uphold all confidentiality standards.
  • Assist case managers and clients with housing applications, searches, and connections.

Preferred Experience/Minimum Qualifications

Required:

  • 2 years of experience in Housing Navigation or Case Management, or a related field.
  • Valid CA Driver's License and eligibility for company vehicle insurance.
  • High School Diploma or equivalent.
  • Strong customer and client relations skills.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams).
  • Knowledge of HUD Housing inspections and reporting.
  • Understanding of the housing rental market and property management.

Preferred:

  • 3+ years of relevant experience.
  • Bilingual in English and Spanish.
  • Bachelor's Degree or at least one year of case management experience or a similar position.
  • Experience in homeless services, case management, mental health, or permanent supportive housing.
