Job Summary

The Outpatient Case Manager is a key member of our team, responsible for assessing, treating, and coordinating care for high-risk members. This role requires strong clinical skills, excellent communication, and a passion for delivering exceptional patient care.

Key Responsibilities
  • Conduct comprehensive assessments of high-risk members to identify their needs and develop individualized care plans.
  • Collaborate with physicians, healthcare providers, and other stakeholders to ensure seamless care coordination and effective communication.
  • Develop and implement plans of care that address the member's medical, environmental, and psychosocial needs.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Communicate effectively with members, families, and healthcare providers to ensure that all parties are informed and engaged in the care process.
  • Identify and address psychosocial needs of members and families, facilitating consultations with social workers and other specialists as needed.
  • Participate in interdisciplinary case rounds to ensure that all members of the care team are informed and aligned in their approach to care.
Requirements
  • Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • Current CA RN or LVN license, current CPR certification, and valid CA Driver's license.
  • 3 years acute care or case management experience preferred.
  • 2-3 years of utilization or HMO experience preferred.
  • Typing 40 words per minute with accuracy.
  • Knowledge of computers, faxes, printers, and all other office equipment.
  • Knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access, and PowerPoint).
